CHI Healthy Spirit offers Condition Management Coaching through Preventure. This can help anyone who wants to feel better and live a healthier life. If you choose to join, you will be matched with a personal health coach who you can talk with on the phone. Your Preventure coach will offer education, tips, and guidance to help you reach your health and wellness goals.

We will discuss your medical history, health goals and any challenges you may be facing. After that, your coach will call you on a regular basis to work on your goals. These calls are set at a time that works best for you: morning, afternoon or evening. Most calls last 15 minutes.

Q: What can coaching do for me?

A condition management coach can help if you have a long term health concern, such as asthma, diabetes, heart disease, heart failure, chronic obstructive pulmonary disease (COPD), or back pain. Your coach can also serve as a bridge to your health care team, including your doctor.

Q: Who are the coaches?

The health coaches include registered nurses, diabetes educators, dietitians, exercise physiologists, respiratory therapists, and other specialists, with an average of 10 or more years of experience. They are people you can trust and rely on.

Q: Why is CHI offering this program?

It’s simple. When you are healthy and feel great, you have the time and energy to get more done at home and on the job. And the healthier you are, the less likely you are to need costly medical services. Your employer wins and so do you.

Q: How much does the program cost?

There is no cost for you or your eligible family member to join and take part in Preventure Coaching. Your employer will cover the costs as long as you are eligible.

Q: What happens after I enroll?

After you sign up, one of the first things we will do is set up a time to talk and get to know you better. The first call will take about 30 minutes. We will discuss your medical history, health goals and any challenges you may be facing. After that, your coach will call you on a regular basis to work on your goals. These calls are set at a time that works best for you: morning, afternoon or evening. Most calls last 15 minutes. How often we talk with you will be agreed upon by you and your health coach at the time of each coaching call. After a call, your coach may send you educational material to help with your health and making healthy lifestyle changes.

Q: When can I reach my coach?

Preventure coaches are available Monday through Thursday from 8am to 8pm, Friday from 8am to 6pm, and Saturday from 9am to 5pm.

Q: I have a special need. Can you help me?

Q: What kind of educational materials will I receive from the program?

You may receive written information about better managing your health and making healthy lifestyle changes. Please let us know if you need information in a different form or in a different language. Preventure Coaching follows best practices when we research, write, and share health information with you. Our award winning materials also follow best practices for health literacy. We may send you emails to help with your health, but only if you say it is OK.

Q: How was I selected for this program? How do you get my information?

The program receives medical and pharmacy data from your health plan and uses HRA and biometric screening data in order to identify people who might benefit from services such as health coaching.

Q: Is my health information shared with others?

No. Only aggregate, or total, results will be shared with your employer. Your employer never knows why you joined or what you discuss with your Preventure coach. The only time your Preventure coach will share the things we learn about you and your health is when you’ve told us it’s OK to do so or as required by law.

Q: How will my information be used?

All information that we collect about you will only be used to provide condition management coaching services and to determine how often we should talk and what kind of goals we may help you set and achieve.

Q: How is my information protected?

All information is treated with the strictest confidence in keeping with the Health Insurance Portability & Accountability Act (HIPAA) and other federal privacy laws that cover the use of Protected Health Information (PHI). PHI is any health information that could be used to personally identify you.

Q: Can I request a copy of my record?

Yes, you may request a copy of your record at any time. Your record includes any actions taken or referrals made to other resources that can help with your health. If you would like a copy of your record, please let your Preventure coach know what information you would like and how we should send the information to you.

Q: How do I file a complaint?

You can call Preventure Coaching at 18444515807 if you have an urgent need, a complaint, a grievance or a concern. We will work to resolve your complaint within 10 business days. If you feel your health information may have been used, disclosed, or shared inappropriately, you or your caregiver may speak with the Preventure Coaching Privacy Officer. Know that our Preventure coaches are not allowed to advertise, market, or promote specific products or services to you or your doctor.

Member Rights and Responsibilities

These Rights and Responsibilities explain what you can expect from our program. They also explains what you can do to maximize your success in the program. We use your information to provide Preventure Coaching services to you. We may also share some information with your health plan or with other service providers. This information can help them answer your questions. It can also help them tell you about other services they may have. Our use of your information will follow the rules set by the Health Insurance Portability and Accountability Act (HIPAA).

What you can expect from us:

What we expect from you:

You have the responsibility to:

  • Be active in the coaching process.
  • Tell your doctor that you joined our program.
  • Follow your doctor’s orders and Preventure Coaching guidance.
  • Give us true information about your health.
  • Tell us about any changes to your health treatment.
  • Let us know if you cannot keep your coaching calls.
  • Tell your Preventure Coach if you decide to stop participating in the program.
